Medication Therapy Management is a service designed to help patients understand their medications, from when they are supposed to take them, how to take each one and which disease the medicine is treating.

Fitchburg Family Pharmacy can help you keep track of the different medications you take on a daily basis and help you understand how each one fits into your healthcare plan.

Furthermore, your pharmacist can answer any questions regarding side effects or interactions that may be harmful including interactions with over the counter medications. These 30 - 60 min meetings are available by appointment only, please use the calendar below to schedule.

Fitchburg Family Pharmacy’s services platform is hosted by WorkFlow Services. You must get a QR Code before your appointment.

All patients must submit patient data into the WorkFlow Services platform.

To complete this profile you will need a copy of your State Issued ID and your medical insurance card and a valid credit/debit card. Your card will be charged for services that are not covered by insurance. You will need to take a picture of both of these items front and back, with your smartphone during the registration process. If using a computer to enter the data, you can type it in.

Once you have completed the online forms, you will be sent a QR code. Please bring that QR code, so we can provide you with the needed services.
