Immunizations

All recipients must schedule an appointment and complete our online consent form. You will be automatically redirected after you schedule your Appointment.

If you are ready to receive your vaccine (6 months+) please click below to schedule an appointment for your vaccine. Then you will be prompted to register for your vaccine.

Please bring your white vaccine card, ID, and Insurance card to the appointment. We have the following vaccines available:

ATTENTION: Any child under 3 years must have a prescription for each dose of a vaccine. The parent or guardian of the child must contact their medical provider who must send a prescription into the pharmacy prior to each dose.

Some of the available vaccines:

• Influenza (Flu) - 6 mo to 64 years
• High Dose Influenza (HD Flu) - 65 years and up
• Flumist (Nasal Flu Vaccine) - 2 years - 49 years
• Respiratory Synsitial Virus (RSV) - 75 years and up
• Respiratory Synsitial Virus (RSV) - Infant dosing (This vaccine is currently unavailable)
• Hepatitis A (Hep A)
• Hepatitis B (Hep B)
• Gardasil (HPV)
• Pneumonia (PVC - 15, PVC - 20, and PVC -23)
• Shingrix (Zoster) - 50 years and up
• Tetanus Diphtheria Pertussis (TDAP)
• Tetanus (Td)

Fitchburg Family Pharmacy’s services platform is hosted by WorkFlow Services. You must get a QR Code before your appointment.

All patients must submit patient data into the WorkFlow Services platform.

(One Time)

To complete this profile you will need a copy of your State Issued ID and your medical insurance card and a valid credit/debit card. Your card will be charged for services that are not covered by insurance. You will need to take a picture of both of these items front and back, with your smartphone during the registration process. If using a computer to enter the data, you can type it in.

Once you have completed the online forms, you will be sent a QR code.
