COVID Vaccinations

All recipients must schedule an appointment and complete our online 

consent form. You will be automatically redirected after you schedule your 

appointment.

If you are ready to receive your COVID-19 vaccine (6 months+) please click below to schedule an appointment for your vaccine. Then you will be prompted to register for your vaccine. Please bring your white vaccine card, ID, and Insurance card to the appointment. We have the following COVID vaccines available:

• Spikevax 12 years and up

• Spikevax 6 mo to 11 years

• Novavax 12 years and up (This is only offered on specific days, watch our Facebook page for announcements)

ATTENTION: Any child under 3 years must have a prescription for each dose of a vaccine. The parent or guardian of the child must contact their medical provider who must send a prescription into the pharmacy prior to each dose.

All patients must submit patient data into the WorkFlow Services platform.

To complete this profile you will need a copy of your State Issued ID and your medical insurance card and a valid credit/debit card. Your card will be charged for services that are not covered by insurance. You will need to take a picture of both of these items front and back, with your smartphone during the registration process. If using a computer to enter the data, you can type it in.
