COVID Testing

We no longer have a PCR Testing Solution Available.

Our team can offer monitored COVID Antigen testing. This test will take about 15 minutes to administer and will notify you via text or email of your results. Results will also be uploaded to the appropriate State Agencies. You will have an official document for your use.

Fitchburg Family Pharmacy’s services platform is hosted by WorkFlow Services. You must get a QR Code before your appointment.

All patients must submit patient data into the WorkFlow Services platform.

To complete this profile you will need a copy of your State Issued ID and your medical insurance card and a valid credit/debit card. Your card will be charged for services that are not covered by insurance. You will need to take a picture of both of these items front and back, with your smartphone during the registration process. If using a computer to enter the data, you can type it in.

Once you have completed the online forms, you will be sent a QR code. Please bring that QR code, so we can provide you with the needed services.
